Thrombolyse et Angioplastie coronaire

REACT : Rescue Angioplasty versus Conservative Therapy of Repeat Thrombolysis, cette première étude démontre l'intérêt de l'angioplastie primaire (PCI) en cas d'échec de la thrombolyse. Le suivi à 6 mois montre la supériorité de la PCI comparée à la thrombolyse et au traitement conservateur. Il faut cependant noter que cette étude a été arrétée de façon prématurée du fait d'un enrolement insuffisant.

Œdème pulmonaire cardiogénique

OAP cardiogénique et VNI : la bilevel-PAP semble équivalente à la CPAP, résultat d'une étude randomisée publiée dans le Critical Care Medicine. Comparée à l'oxygénothérapie standard, la VNI diminue les indication d'intubation, améliore le rapport PaO2/FiO2, la FR, la FC sans augmenter l'incidence de l'IDM. Les investigateurs suggèrent que la VNI en pression positive doit être un standard de traitement.
